Diagnostic Questions      None at All / A Little / Average  / Strong / Very Strong

1. Feel nervous, unsettled

2. Get easily distressed, angered

3. Feel depressed, emotionally down

4. Feel you’re not as good as others

5. Sleeping poorly, e.g., having trouble getting to sleep, easily woken, or waking up early

★ 6. Have had thoughts of suicide.

Total points from questions 1-5

Explanation of Point Totals:

0-5: Mental health in good state

6-9: Small degree of emotional distress. Would suggest looking for ways to ease stress, or seeking guidance from a psychological counselor.

10-14: Medium level of emotional distress. Would suggest seeing a psychiatrist for further evaluation.

15 and above: Serious emotional distress. Would suggest seeing a psychiatrist for further evaluation.

★ Question 6  (Have you considered committing suicide?): This is an added question. If your point total on the first five questions was lower than 6, but you had a 2 or higher on this question, you should still consider seeking out a professional psychiatrist.

Note: This test is called a Brief Symptom Rating Scale (BSRS-5). It was developed by Ming-been Lee and colleagues.
